Preparing for Rhinoplasty
First and foremost is a consultation with Dr Rahul Gogoi. He will prepare you and make you understand the surgical procedure. What is the realistic expectation as every nose is unique and individual as you yourself. Once you are sure that you would like to proceed with the rhinoplasty surgery. We will be guiding you how to prepare for surgery under local anaesthesia as well as surgery under General Anaesthesia
Administration of Local Anesthesia: Dr Rahul uses a small needle of 30 gauge diameter to inject a local anesthetic, made up of combination of Prilocaine , lidocaine with epinephrine, into the specific areas of the nose that require numbing. The epinephrine constricts blood vessels, reducing bleeding during the procedure and allowing for better visualization of the surgical site.

Type of rhinoplasty incision / approach:
After the anesthesia has taken effect, trhinoplasty begins with making small incisions in inconspicuous locations. These incisions may be made inside the nostrils (closed rhinoplasty) or along the columella (the strip of tissue between the nostrils, in an open rhinoplasty). The choice of incision technique depends on the specific goals of the surgery and the internal structural shapes and support
Once the incisions are made, Dr Rahul carefully lifts the skin and soft tissues, to visualize the nasal framework. Following this Dr Rahul modifies the nasal bones, cartilage, and other tissues to achieve the desired changes.
The specific techniques used to reshape the nose vary depending on the individual case. Common procedures include:
-
Cartilage modification: it involves trimming, reshaping, or reposition the nasal cartilage to refine the shape of the tip or bridge of the nose.
-
Osteotomy: In some cases, the Dr Rahul may need to perform osteotomies, which involve carefully breaking and repositioning the nasal bones. This technique is used to correct issues such as a crooked nose or a wide nasal bridge. Dr Rahul Specialises in using advanced Piezo surgery to cut the bone precisely. Dr Rahul is one of the few surgeon in the world to use ultrasonic rhinoplasty in all his nose surgeries
-
Septoplasty: If there is a deviated septum (the wall that separates the nasal passages), a septoplasty may be performed to improve breathing and correct any structural abnormalities.
-
Tip refinement: Dr Rahul uses various suturing techniques or grafts to refine and reshape the nasal tip, creating a more aesthetically pleasing appearance.
Closure and Recovery: Once the necessary changes have been made, Dr Rahul carefully repositions the skin and soft tissues back over the nasal framework. The incisions are then closed with dissolvable sutures, and a splint or external tape may be applied to support the new shape of the nose and help minimize swelling.
After the surgery is complete, you will be monitored in a recovery area for a short period to ensure you are stable. One may experience some mild discomfort, swelling, and bruising in the days following the procedure, but these symptoms can be managed with pain medication and cold compresses.
Dr Rahul will than provide detailed post-operative instructions, including how to care for the surgical site, what medications to take, and when to attend follow-up appointments. It is crucial for you to follow these instructions to promote proper healing and achieve optimal results.
Postoperative Follow-up: You will typically have several follow-up appointments with the surgeon in the weeks and months following the surgery. During these visits, Dr Rahul will monitor the healing process, remove any sutures, assess the results, and address any concerns or questions the patient may have.
Over time, the swelling will gradually subside, and the final results of the rhinoplasty will become more apparent. It is important to note that the complete resolution of swelling and the achievement of final results may take several months, or even up to a year, depending on the individual.
Rhinoplasty What to expect?
Dr Rahul Gogoi has one of the highest number of rhinoplasties under local anaesthesia. The surgery can last anywhere between 4-6 hours in duration. The local anaesthetic solution used is Tumescent Anaesthesia the effects of which last for about a duration of 8 hours.
1. Post Surgery nasal taping and thermoplastic cast will be in place for a period of 10 days
2. Nose stitches and cast are removed on the 10th post operative day
3. Night taping has to be done to help the skin and tissues adapt and re-attach to the internal cartilage framework for a period of 3-4 weeks
4. The total duration of complete resolution of the swelling of the nose can be between 6-12 months
